CDC Issues Level 3 Travel Alert As 'Largest Ebola Outbreak In History' Spreads
Things appear to be going from worse to worst as the deadly Ebola epidemic surges on. The CDC has issued a Level 3 - Avoid All Non-Essential Travel - warning.
CDC urges all US residents to avoid nonessential travel to Liberia, Guinea, and Sierra Leone because of an unprecedented outbreak of Ebola.
An outbreak of Ebola has been ongoing in Liberia since March 2014.
This outbreak also affects Sierra Leone and Guinea; to date more than 1320 cases have occurred in the three countries and more than 725 people have died, making this the largest outbreak of Ebola in history.
At least three Americans have been infected; two are health care workers in an Ebola clinic. Affected districts include Bomi, Bong, Grand Gedeh, Lofa, Montserrado (including the capital city of Monrovia), Margibi, and Nimba.
Instances of civil unrest and violence against aid workers have been reported in West Africa as a result of the outbreak.
The public health infrastructure is being severely strained as the outbreak grows.
CDC to send additional 50 experts to region in next 30 days.
Dr. Thomas Frieden, director of the Centers for Disease Control and Prevention, said Thursday that even in a best-case scenario, it could easily take three to six months to stem the epidemic in West Africa.
Do not worry though, as the US reassures there is no risk...

Oh absolutely. In fact the chance of ebola becoming vastly more dangerous just went up a lot ... because ebola has "learned" how to reach an urban environment. To now ebola only naturally resides in the countryside. People contract the disease by eating bushmeat (monkeys). After that, it "travels" into cities by suddenly acquiring particular traits that lend it to travel. So for example, being a little less lethal, or having a longer refractory period, or spreading a little easier. These are all the traits of a bug that will be able to randomly leave the countryside and be found in distant cities.
Being able to reach cities is important.
There are many more people, yes, but the virus doesn't see it as people. We are just one huge petri dish the virus can grow in. What it notices however is that the food on which it will feed (that's not the right description, but roll with it) is suddenly much more diverse and interesting. And there are vastly more other type of pathogens present to interact with and "learn" from. That, BTW, is how bird-flu jumps into humans, by learning something from other viriuses that already infect people, but it can only learn that when there are a lot of people with all sorts of different immunological profiles, and alrady carrying a huge number of familiar and non-lethal viral loads.
So the city is the ideal place for ebola to "take it up a notch" on the great scale of scary things.
And that is why I'm a little worried now.

The reason for the low mortality rate is supportive therapy - particularly IV fluids. As the helath infrastructure is now getting overwhelmed, I expect mortality rate will increase. We'll see.
It might also be that the virus has become slightly less lethal.
If true, that is not good news. One of the things that keeps ebola from spreading is that it outright kills people really fast. It used to be they got sick, and within a day or two they were dead where they lay.
If ebola has learned a new trick, it might be "laying low" for an extended period looking like tiredness or common cold. This allows people to move around -- travel, visit family, take a business trip, fly home -- potentially spreading infection as they go, but certainly leaving the area of epidemic and causing new outbreaks.
Microevolution dictates that pathogens take the infectious route that best propagates them among hosts. For something like ebola, tuning it down a little is actually highly adaptive; it becomes a ninja virus. It need only learn one more trick -- how to become aerosolized via coughs and sneezes -- to make it a very dangerous infectious agent with global implications.
I suspect it is nearly there. What we are seeing in Africa is the spread of something highly lethal but also very mobile, that is taking seasoned professionals by surprise.

The long incubation period (up to 21 days) makes it far easier for the disease to spread than a disease which shows more immedate effects. Right now there are likely dozens of people who are unknowingly infected and have left the quarantine areas. When they finally do fall ill and become contagious they will be exposing whole new areas to the disease. An infected individual could easily be in Europe already but they haven't developed symptoms yet so their presence is not known. Early care givers risk becomming infected because they don't know what they are dealing with. Fully clamping down on the outbreak will be very difficult if governments don't limit travel to and from those countries with active outbreaks.

INDICATIONS emerged, yesterday, that one of the 15 airport contacts that assisted the late Liberian, Mr. Patrick Sawyer, that died of Ebola Virus Disease, EVD, may have contracted the deadly virus.
Unconfirmed reports revealed that already the contact has begun to manifest symptoms of Ebola Virus.
According to a member of the Joint Federal and State Team on Ebola outbreak who preferred anonymity; one of the persons who accompanied late Patrick Sawyer is already showing signs of Ebola.
The reliable source confirmed receipt of a short message service, SMS, text that the person who assisted late Patrick Sawyer to move out of the plane is presenting symptoms, identifiable with EVD. "In fact, I just got a text that the person who helped Sawyer, the Liberian, to get out of the air plane is down with a suspected case of Ebola. You can see we have to be very careful. I cannot say specifically whether he or she is a Nigerian or in Nigeria right now. He is still being monitored as we speak," he said.
He added that the latest victim was among those who were being quarantined for having first contact with the victim.
Meanwhile, the Lagos State Government in a swift reaction through a text message said that no one of the 59 contacts is sick for now. The Special Adviser to the Governor on Public Health, Dr. Yewande Adeshina who responded through a text message said: "It is not true. We are doing routine surveillance, no one is sick."
It would be recalled that the Lagos State Government, Monday, disclosed that not less than 59 persons that had contact with the victim of Ebola Virus Disease, EVD, Patrick Sawyer, who died in Lagos last Thursday have been identified and that the Joint Federal and State Team in collaboration with other health organisations on prevention of Ebola outbreak in Lagos, have begun close monitoring of the 59 persons.

Not impressed with the source. If the 30k number is an estimate of all the people the guy might have walked past in 4 airports then that says something about contracting the disease that does not align with our understanding of the bug. If someone thinks ebola has become airborne then that's big news -- really huge news -- that I don't see being reported and that does not align with how the virus is seen moving around between known victims, who so far have all been in places where physical contact was likely.
Now, if we see a housewife in Colorado come down with ebola, who only happened to be in Africa for a laid-over flight on her way from somwhere else -- then we have a big big problem. At the moment, not so much.
Though yes if that housewife can only point to having walked passed a bunch of Africans in an airport, then 30K is by no means a large number and in fact I'd think globally more like 300K.
At this level I think it is a true test of the risk. If it doesn't turn into a massive pandemic, then it isn't as bad as we think. But if it does, well, let's just hope it doesn't.

My point was just that even though 250k die from the flu, it's a small proportion of the total infected. IF Ebola became mutated into something more contagious, while maintaining its virulence (and I don't see that there's any selection pressure to encourage evolution towards a lower-mortality strain) the numbers would be bad in a way the world hasn't ever experienced.

As for ebola maintaining its virulence after mutating, it would probably have to do so under conditions that hold some similarities to the conditions that gave rise to the Spanish flu. Also, as Cougar said somewhere else in this thread, becoming a tad less deadly could be bad. I can see something that has a 25% mortality rate spreading a lot further than something that has a 90% mortality rate. Becoming contagious BEFORE symptoms appear would also be bad news.

It could wipe out a large proportion of the populations of these countries. It is not as if they can vaccinate or do anything like that. They can't do anything about it but quarantine but who do they do that to and where? In addition local customs and ignorance seem to accelerate the spread of the virus. Some of the early carriers just ran away from hospital when they found out.
The world has not seen anything like this since Spanish flu in 1919 when millions died. They didn't have low cost mass international travel then either.
Those countries in the West with large populations from West Africa, who may be regarded as safe sanctuaries, are particularily at risk.
With contagion via bodily fluids and short range in the air (sneezing) this will be hard to stop.

On the flights from South Africa to the US several airlines have to refuel in Dakar, Senegal, before continuing on to the US as they are going westbound against the winds. Although the federal government's official reaction to all this is low-key, in truth the U.S. government is rapidly preparing for the possibility of an Ebola outbreak reaching the continental USA.
As reported above, the U.S. Department of Defense already has a $140 million contract awarded to Tekmira for its Ebola treatment drugs.

The Department of Defense informed Congress that it has deployed biological diagnostic systems to National Guard support teams in all 50 states, according to a report published by the Committee on Armed Services. Some 340 Joint Biological Agent Identification and Diagnostic System (JBAIDS) units have thus far been given to emergency response personnel. The systems are "rapid, reliable, and [provide] simultaneous identification of specific biological agents and pathogens."
On one hand, we might all applaud the government's preparedness actions in all this. It's smart to have diagnostic systems deployed nationwide, of course. But it begs the question: When was the government planning on telling the public about all this? Probably never. There's no sense in causing a panic when half the people won't survive an outbreak anyway, they figure."
