Ebola Deaths Go Exponential; Nigeria Demands Experimental Drug From US, Saudi Death First In Arab World

The official Ebola death toll is now at 932 with over 1,700 reported cases but as the WHO reports, in the last 48 hours, deaths and cases have exploded (48 and 108 respectively). As the charts below show, this epidemic is going exponential. What is perhaps most worrisome is, while playing down the threat in Nigeria (most especially Lagos - which the CDC Director is "deeply concerned" about), officials have formally asked the US for the experimental Ebola drug, which suggest things are far worse than the 3 deaths reported so far in Nigeria would suggest. Finally, as we warned yesterday, Saudi Arabia is suffering too as the main who was hospitalized yesterday with symptoms has died - the first reported casualty in the Arab world.

 

As The BBC reports,

It is the world's deadliest outbreak to date and has centred on Guinea, Liberia and Sierra Leone.

 

Eight people are currently in quarantine in Nigeria, Africa's most populous country, and two have died there.

 

 

 

The Saudi man died after showing Ebola symptoms when he returned from a business trip to Sierra Leone, the Saudi health ministry said.

Nigerian Health Minister Onyebuchi Chukwu has sent request to director of U.S. Centers for Disease Control and Prevention, he tells reporters in capital, Abuja.

 

“We have been in communication in the last 36 hours”

 

“We are getting reports that the experimental drug seems to be useful”

Which suggests things are far more serious in Nigeria than officials are letting on. As The BBC reports,

But it is not clear if the ZMapp drug, which has only been tested on monkeys, can be credited with their improvement.

 

Prof Peter Piot, who co-discovered Ebola in 1976, Prof David Heymann, the head of the Centre on Global Health Security, and Wellcome Trust director Prof Jeremy Farrar said there were several drugs and vaccines being studied for possible use against Ebola.

 

"African governments should be allowed to make informed decisions about whether or not to use these products - for example to protect and treat healthcare workers who run especially high risks of infection," they wrote in a joint statement.

 

The WHO, "the only body with the necessary international authority" to allow such experimental treatments, "must take on this greater leadership role", they said.

As CDC's Director warned...

As Al Arabiya reports,

A Saudi man, who was hospitalized for suspected Ebola infection, died on Wednesday.

 

The man, in his 40s, had returned recently to Jeddah from a business trip to Sierra Leone, the Health Ministry said on Tuesday. He was admitted to hospital on Monday and showed symptoms of Ebola virus infection.

 

His death marks the first reported casualty of the Ebola epidemic in the Arab world and comes as an emergency World Health Organization summit was being held in Geneva to discuss measures to tackle the epidemic.

Why not simply isolate a small amount (nanograms) of the protein antibody, clone it and amplify it using standard methods?  You know, the old monoclonal antibody trick we've been using to make antibodies for everything under the sun for the last decade or so.

Yep, it was immediate quarantine under pain of summary execution, followed immediately by physically sealing up the home with all family members inside.

There was also a few days' quarantine period for all travelers entering cities and towns, outside the gates of course, in case any were infected but not symptomatic yet.

However,such measures did not work, they failed completely, mostly because no one had the slightest idea about germ theory and contagion.

The only thing that worked was immediately bugging out to your remote and isolated villa for a few years.

The data do not suggest an aerosol transmission. Number of infected people would be much higher. Of course we may need another week or so to see what is really happening in Lagos and other cities.

More interesting to watch death rate as infection spreads. Currently 55% or so, will it burn out, will it go nuclear, will it keep going and make (continued) life a coin toss?

Not sure I'm investing in a P100 mask at this point, but maybe just to put it on eBay.

Take my calculations with a grain of salt, I am ony the best fucking analyst around here.

Taking near patient zero, one P. Sawyer (Patrick, not the more notable Pine Saywer of the Michodamus beetle family which has fuck all to do with this as of now), 14 patients have been quarantined post his death. Of those, 8 are showing symptoms and 1 is confirmed newly ebola wealthy.

Whether you go with the 8 or the 14 makes no difference really. Vector extrapolated against incubation period and dispersion provides for 100% global exposure ("OhFuck">7 billion) at 18-26 weeks. Meaning everyone but me and my tribe of cannibals in the seychelles will have breathed a little in by Superbowl Sunday 2015.

If it's 14 per incident that's closer to Thanksgiving of this year. Closer to 1 and it only buys about 3 more months. Exponential growth curve bitchez, it's the last double that pays for your lunch or eats it.

No mention that it takes a certain threshold of virus particles to actually be infected. Just one isn't going to do it, folks. But no, that would be *gasp* talking calmly and logically about things.

While I question the decision to bring those patients back, it isn't like the damn thing is going to go airborne. If it had, you'd be seeing the results already, believe me - on the order of, your neighbor is suddenly sick, and maybe your kid/spouse/whatever.

You sure as hell wouldn't be sitting here and posting on ZH.

I want to get behind this line of thinking for my own sanity, but the doctors that were infected took every procaution imaginable and were still infected.  That does not sit well with me.  I don't think the doctors there are having promiscuous sex with the locals or going out to eat at the local bush meat establishment, but maybe I'm wrong.

What about the common sense idea that the folks who treat this thing are physically present in the same room with the disease much more than the rest of us? In other words they have to roll the dice regarding exposure much more than the average homo sapien.

The radio dude where I am is being sure to tell people that the doctors who contracted this and were flown to Atlanta did NOT have protective gear on and that is why they got it.  This is after some caller asked how they are getting ebola with protective gear on.

I am asking how either one of them knows for sure what the hell happened.

Nobody with any medical/epidemiological training who has been educated about Ebola is going to intentionally contact an infected person without full protective gear and decontamination support.  

Also, there is no way all of these health care workers are the victims of accidental needle-sticks or suit ruptures. . .

  The thing that I find most disturbing is that the healthcare workers knew they were dealing with a notoriously deadly virus, and likely took appropriate precautions, but still approximately 100 of them became infected.  If this strain of ebola can only be spread through direct contact with a good portal of entry (ie. cut, mouth, etc.), then those infected would have to be incredibly careless to contract it. 

  Unlike most, I have provided direct care for patients with a wide variety of communicable diseases (HIV, hepatitis, shingles, C. difficile, TB, etc.) for over 18yrs, with only gowns, masks, gloves, and hand washing to protect me.  I remain disease free after all these years.  Are we to believe that those doctors, nurses, etc. were that careless?

There are two types of airborn virus vectors, wet, and dry. One is fine droplet as in a sneeze, but in theory loose dry virus strands floating in the air by themselves either after the droplet has evaporated or by being stirred up from an already dried state can also be a vector. Scary if that is the case with the infected health care workers. If this is the case what is the filter size needed for this virus.

If the virus can only be spread by bodily fluids why have so many doctors (and nurses) contracted it? 

Very possible it has mutated and can be caught through aerosol transmission (cough/sneeze).

The mutation would take time to be confirmed; meanwhile, inbound flights from Africa daily.

My Daughter lives near the D.C. area, I'm not too happy about 50 African Heads of State and their entourages being hosted in D.C. this week.

While fruit bats are considered natural reservoir, involvement of other species in EBOV transmission is unclear. In 2009, Reston-EBOV was the first EBOV detected in swine with indicated transmission to humans. In-contact transmission of Zaire-EBOV (ZEBOV) between pigs was demonstrated experimentally. Here we show ZEBOV transmission from pigs to cynomolgus macaques without direct contact. Interestingly, transmission between macaques in similar housing conditions was never observed. Piglets inoculated oro-nasally with ZEBOV were transferred to the room housing macaques in an open inaccessible cage system. All macaques became infected. Infectious virus was detected in oro-nasal swabs of piglets, and in blood, swabs, and tissues of macaques. This is the first report of experimental interspecies virus transmission, with the macaques also used as a human surrogate. Our finding may influence prevention and control measures during EBOV outbreaks.

The study suggests that pigs can transfer the virus without contact, but macaques can not.  No info about humans.

Survivors should have lots of antibodies in their serum and should be donating to the newly confirmed cases in africa.

Y'all better pay heed. My wife's clinic has given no warnings; network of 7 states. She asked her boss about it and she literally said, "what's Ebola?" Get P100 masks and gloves(good to have anyway). Our friends in the Bronx ER have not seen any people with signs, but The Sinai patient not having a test back is highly suspect. My cousin the epidemiologist at Emory is going to be contacted by my wife on Friday. We want her to have time for a full report. Keep ya posted. Prepare. Wife has a bad feeling and she is pretty chill.

My wife is a molecular biologist (PhD) who also has a MS in Epidemiology - she is watching this shit like a hawk too.  

There just haven't been enough Ebola outbreaks to be confident how a large-scale outbreak in an urban area will spread.  This event is an unknown quantity that could go any number of different directions - but on the off-chance that this outbreak does go global, it makes sense to keep on top of things and be as prepared as you can be.

I'm a worrier by nature, that's why I hang here for threat level, but this is scary shit cuz it's proving to seem airborne. With it's kill rate around 55-60% and in a city like Lagos or NYC, shit could get very bad very quickly.
