You know, a few days ago, I wasn't worried about an outbreak in Dallas and, I'd still like to think that at least this set of patients would we contact traced and we'd get this particular set of cases stopped.

I'm not sure it's stopped now....  Hopefully it will be soon.

How is the 3rd world going to deal with this....    I guess all we have to do is look to Africa to see....  The virus is spreading exponentially....  Even in Dallas, the patient infected 2 workers ( so far )...   it proves how deadly this thing is....   It may already be too late to do anything about it in Africa....

This thing is beginning to look like Capt. Tripps.

The focus for right now has been on hospitals. But I gotta tell ya I'm envisioning dentists and all their blood-handling personnel are in almost the same hazardous environment. Maybe even more so when you consider the 'misting' drills etc., project out.

The dog belonging to the Dallas nurse infected with Ebola has been moved to quarantine in a decommissioned naval base. Nina Pham was happy to hear Bentley was doing just fine. Jane Pauley reports.

This is the text from a document my wife received from her hosptial's infectious disease group.  She is an ER physician at a level 1 trauma center in central FL.

 

Ebola Screening: Exposure concern: ALL body fluids.

 

1. Emergency Department-

Triage RN screening question: Have you been to West Africa in past 21 days and do you have a fever or other 

 

symptoms such as severe headache, muscle pain, vomiting, fatigue, unexplained bleeding or diarrhea? If yes, don 

 

N95 mask, goggles, isolation gown, and double gloves and place patient in negative air flow room, implement care, 

 

and call Infection Prevention Clinician on-call. If no, but been to West Africa, further screening will occur by ED 

 

RN/physician. 

 

2. Admitting Department-

Main admitting, decentralized registration, and ambulatory sites

 

Ask patient: Have you been to West Africa in the past 21 days? If yes, place patient in a private room, and notify 

 

Rapid Response Team to evaluate for fever or other symptoms as listed above. Document response in the 

 

documents tree in the Registration pathway. Rapid Response team will wear N95 mask, goggles, isolation gown and 

 

double gloves to assess for fever and symptoms. If fever or other symptoms present, place patient in blue isolation 

 

gown, mask with eye shield, call ED to prepare room and transport to the ED. ED will notify Infection Prevention.

 

3. Transfer Center:

 

Ask all transfers in “has the patient been to West Africa in the past 21 days?” If yes, tell them you will have 

 

Infection Prevention call them back in a few minutes. Call Infection Prevention and document in the documents 

 

tree in the Registration pathway.

 

4. Phone scheduling/registration

 

Ask all patients being registered or scheduled on the phone: “have you been to West Africa in the past 21 days?” 

 

If yes, tell them you will have Infection Prevention call them back in a few minutes. Call Infection Prevention and 

 

document in the documents tree in the Registration pathway

 

5. Nursing

 

Two Infection Prevention Screening questions are in the Admission Navigator: 1) Travel history to West Africa 

 

in past 21 days; and 2) Travel history to West Africa in the past 21 days and has a fever of 100.4 or symptoms 

 

including severe headache, vomiting, abdominal pain, diarrhea, unexplained bleeding or muscle pain. If patient 

 

has fever or symptoms plus travel history to West Africa within past 21 days, BPA will alert to place patient in Strict 

 

Isolation and call Infection Prevention immediately. 

 

i. Don N95 mask, goggles, isolation gown, and double gloves 

 

ii. Place patient in private negative airflow room with anteroom – specifically K 5431 or 5437 would 

 

be first choice, followed by K5110 or 5114, and 3rd

 

iii. Use Dangerous Contagious Disease Contact form- Appendix A to have someone record names and 

 

contact information of anyone who has had contact with the patient- this can be found on the 

 

portal included in the policy- IC 33

 

iv. Order Ebola cart from SPD 

 

v. Do not allow visitors to enter room

 

vi. Do not allow removal of any supplies, trash or linen until cleared by Infection Prevention

 

General Instructions for Care: 

 

1. Place in anteroom in ED K2301 and K2302, Neuro ICU K5431, K5437, K5453, K5457, Trauma ICU K 5110, K5114

 

2. Notify Infection Prevention upon arrival to room and Supervisor/Manager who will notify NAOC. 

 

3. Order Ebola Cart from SPD

 

4. Assign one RN to the patient for the shift. RN to take care of no other patient. 

 

5. Infection Prevention will assist with donning and removal of personal protective equipment.

 

6. Wear yellow jump suit, N95 mask, face shield, shoe covers and double gloves when entering room. 

 

7. Security and Infection Prevention to control access to room. Limit care givers. 

 

8. Leave all trash in the room until further guidance provided. 

 

9. Patient to remain in room for all testing. 

 

10. Infection Prevention to be on the unit during patient admission to provide guidance (24/7). 

 

11. Once patient leaves ED room or discharged, contain room without anyone to access, until cleared by Infection 

 

Prevention.

I've learned so far there are only twenty (20) class-4 confinement rooms in the USA with negative air pressure (used years ago for TB patients).  There are only two (2) at the VA's.  The only way to confine would be a 'camp' type onfinement or "Logan's Run" style acid bath/meltdown and mop-up operation (once you are Ebolized).

Please allow me to point out the OBVIOUS in that the CDC should have been all over that hospital like stink on shit the first moment Ebola was confirmed.  They should then have IMMEDIATELY transferred him to a Bio-Safety Level 4 facility wherein positive pressure whole body suits could be worn by any caregivers.  The talk about treating this viral outbreak in open air hospitals is INSANE.  Pure LUNACY!!!!!

CDC protocol calls Ebola a level 4 contagion - bsl4 - there are only 19 beds in the entire us that are bsl4 level.
So what the CDC is saying is it is ok to break their own protocol and treat a level 4 contagion in a bsl2 level 2 isolation ward - 

So we follow protocol Only until something really bad happens then we ditch it? 
That makes ZERO sense - seems like they should be More vigilant not less -

Unfortuneately Dallas medical personnel were given inadequate mandates by CDC when Duncan was first identified as an Ebola patients.  There was no mandate for plastic head coverings and hazmat suites.   CDC revised their protocols AFTER Dallas medical personnel were exposed. 

Dallas is doing a good job of controlling damages caused by the CDC's incompetences.
