Second Texas Healthcare Worker Tests Positive For Ebola

And then there were three.

As the Texas Department of State Health reported an hour ago, just three days after the shocking announcement that the late Thomas Eric Duncan had managed to infect one nurse in the first person-to-person transmission of Ebola on US ground, a second Texas Health Presbyterian Hospital worker has been infected with the deadly virus.

Oct. 15, 2014

 

A second health care worker at Texas Health Presbyterian Hospital who provided care for the first Ebola patient diagnosed in the United States has tested positive for the disease.

 

The health care worker reported a fever Tuesday and was immediately isolated at the hospital.

 

Health officials have interviewed the latest patient to quickly identify any contacts or potential exposures, and those people will be monitored. The type of monitoring depends on the nature of their interactions and the potential they were exposed to the virus.

 

The worker was among those who took care of Thomas Eric Duncan after he was diagnosed with Ebola.

 

The preliminary Ebola test was run late Tuesday at the state public health laboratory in Austin, and results were received at about midnight.

 

Confirmatory testing on a separate specimen will be conducted by the Centers for Disease Control and Prevention in Atlanta.

 

Ebola is spread through direct contact with bodily fluids of a sick person or exposure to contaminated objects such as needles. People are not contagious before symptoms such as fever develop.

Surely the CDC's Tom Frieden will say this is perfectly expected: after all the CDC, which promised it would "stop Ebola in its tracks in the US" just two weeks ago, changed its tune and said to expect more cases.

As of this moment, equity futures are not happy, and certainly not transports, which after a seesaw session in which airline stocks rebounded following a deep oversold condition, are about to become even more oversold as the US public responds to increasingly unpleasant Ebola pandemic news by deciding to "just stay home this weekend."

And now we await the CDC to explain how this is all due to another breach in protocol for a disease which still is clearly not airborne.

Oddly, his Wiki entry does not carry any real personal information such as birth place or date, religion, if he is a dual national, who his father was, it does say who his brothers were. I searched high and low and was able to find [outside of Wiki] that he was born in 1960 in upper Manhattan. Unable to find religion, nationality as to dual, mother's name, only eduation and what he did professionaly, and that he is a Democrat. I did find some tidbits, he worked as a "community organizer" before attending medical school, before and after graduating from Columbia with a master's degree in 1986, he went on medical missions to Nicaragua in conjunction with the Nicaraguan Ministry of Health. This means he worked directly with the Sandinista government.

So we have a possible dual national who is trying to scrub his early history as a communist agitator.

This is one of the finer sides of the "corporations are people" imagining.  Corporations are better than people in this respect.  If I were to say, kill someone "accidentally" because I'm an ignorant dumbass (this happens with cars alot), then I can be tried for manslaughter and go to prison (specially if ah've been drankin').  This almost never happens when a corporation is negligent--it's usually fines and lawsuits.  

As you say, they didn't provide a safe work environment. I would imagine they will be sued by whoever got it from that exposure.  

Sad that it takes tragedy to reveal just what a bloated turd our healthcare system is--costs go up 5+% a year and they are apparently run by a bunch of jokers.  Who would have thought in a business that profits from an inherent form of extortion:  Hey you don't want to die, do you?

Government's handling of this isn't any better.  It's all the same people anyways.  Was wondering what bad news was coming after yesterday's news conference announcing the formation of the "E-Team."  I'm sure that nothing will go wrong when a disease that has been amazingly sucessful at passing to healthcare workers is being treated by people that are being helicoptered all over the country.  

Why would a for-profit company even treat an ebola case willingly?  I won't be surprised when Presby is closed, as it is now the Ebola Hospital as far as the US is concerned. Maybe they are holding out till more people get it.  Then--WINDFALL PROFITS.  And that's what really matters.

Of course, the protocol is probably the size of a telephone book. So as long as they followed every line of a book they have most likely never seen.....they should be sucessfull.

After they got Duncan admitted and went home, the step daughter sprayed the whole apartment with a Clorox solution. She would not let her mother sleep in the bed. That Liberian woman may be more effective at protecting her own than the entire CDC is at protecting us. The daughter was the one who told the EMTs to go back and get protective clothing before they entered the apartment.

This tidbit of info (clorox spray) might be life-saving (albeit messy). I currently use professional chlorox wipes (with actual chlorox, not just the brand name) in my car after I walk through the dried-vomit stained and bloody-tissue littered major metropolitan center city hospital garage.....not that we have any OFFICIAL ebola cases, mind you. Something is very wrong here. Staff on that nursing wing should not drink water out of the nursing station dispenser or eat donuts brought by 'staff' that day. nature of the disease after a fever begins.

I totally agree with sending U.S. soldiers over to Liberia as the infection rate
and mortality is becoming exponential in nature and a military presence with
CDC, WHO supervision is needed to break the infection rate.

This IS NOT the time to open our borders (again) and say “give us your
tired, your poor, your EBOLA masses yearning to get sick on us!

If left alone and not allowed to travel out of infected countries, the plague would resolve itsself over time. At a 20-1 care ratio over 21 days, the system could very well be overwhelmed and abandoned quickly, but we didn't see that coming because the accountant is still crunching the #'s on the odds.

doctors and trained medical staff should treat the ebola patients.  i'm not sure that young kids soldiers have the temperment and the training to do this work.  To do sterile feild work you have to move very by the numbers,  every time, this hand does this, you put this glove here, you put your finger here without touching the other side.  If you make a mistake you could put your patient at risk.  With ebola the consequences are more severe.

With the current CDC safety protocols for hospital workers dealing with possible & confirmed Ebola cases, US healthcare system is has been duped.

Hospitals in the U.S. Get Ready for Ebola says CDC:
  The C.D.C. says that health care workers treating Ebola patients need only wear gloves, a fluid-resistant gown, eye protection and a face mask to prevent becoming infected with the virus. That is a far cry from the head-to-toe “moon suits” doctors, nurses and aides have been seeing on television reports about the outbreak.
Some hospital officials are skeptical of the new advice. “It’s not going to be enough for my health care workers to feel comfortable going into an isolation room,” said Peggy Thompson, the director of infection prevention at Tampa General Hospital.

In contrast to this statement from the CDC, here is what the U.S. military has been saying since 2005 about dealing with Ebola and other forms of Hemorrhagic Fever:

VIRAL HEMORRHAGIC FEVER
Isolation and Decontamination:

All VHF patients should be cared for using strict contact precautions, including hand hygiene double gloves, gowns, shoe and leg coverings, and faceshield or goggles. Airborne precautions should be instituted to the maximum extent possible. At a minimum, a fit-tested, HEPA filter-equipped respirator(such as an N-95 mask), a battery-powered, air-purifying respirator, or a positive pressure supplied air respirator should be worn by personnel sharing an enclosed space with or coming within six feet of a VHF patient. Multiple patients should be cohorted to a separate building or a ward with an isolated air-handling system. Ideally, VHF patients should be isolated in a negative pressure isolation room with 6-12 air exchanges per hour. Environmental decontamination is accomplished with hypochlorite or phenolic disinfectants.

Health workers need optimal respiratory protection for Ebola
We believe there is scientific and epidemiologic evidence that Ebola virus has the potential to be transmitted via infectious aerosol particles both near and at a distance from infected patients, which means that healthcare workers should be wearing respirators, not facemasks.
The minimum level of protection in high-risk settings should be a respirator with an assigned protection factor greater than 10. A powered air-purifying respirator (PAPR) with a hood or helmet offers many advantages over an N95 filtering facepiece or similar respirator, being more protective, comfortable, and cost-effective in the long run.

good point.  hospital isolation rooms are expected to have filtered or segregated ventilation systems.  it woul be unfortunate for an ebola patient to be in a regular room and contaminate the whole hospital.

Not true. The initial air for pressurizing the cabin is tapped off of engine compressor bleed air (which is very hot) and the outflow valve located in the fuselage retains the air as necessary to maintain cabin pressure. The air handling units, located downstream of the engine bleed, serve to cool the bleed air by routing outside cool air around the bleed air tubes as called for by the cabin temp controller, usually pilot controlled. There are recirulating fans that serve to move air inside the cabin around. They also direct the air through either the gaspers (that nozzle above your head in the cabin) or through overhead vents in the ceiling. They do go through air filters at some point in the recirulating system but I doubt that these filters would be considered adequate for the purposes of ebola containment. So no, air in the cabin does not go back through a device that heats the air to 500 degrees. The good news is that at cruising altitude the cabin is fairly "leaky" due to pressuization that is around 9lbs per sq inch on the fuselage walls, so that the air in the cabin is constantly being replaced by outside air. I don't recall how rapidly this actually occurs but it is "frequent".

There are some airframes that have auxillary electric heaters to augment cabin air temps, I'm not familiar with which models have these. I don't believe that all the air is routed through these heaters nor would I know what temp they would heat the air to. They usually would "mix" the air with the ambient inside air to increase the temp, as opposed to routing all the air through them. Either way, the systems aren't set up to "sterilize' air in the cabin.

I agree on this one.  The family is out of sight and out of mind for a reason.  If just one of those quarentined children have died of Ebola, every parent of school age children in Texas become dangerous, unpredictable rioters.  But the CDC is running out of time.  Eventually they will have to announce the family is 21 days clear and healthy, or 10 days dead and cremated.

At least 125 people were being monitored yesterday.  And these are people who had contact with Thomas Eric Duncan. What about people who had contact with Nina Pham and the other healthcare worker?  That list keeps growing.  There will be more cases. Nina Pham's boyfriend is being monitored (rumored to have Ebola) and he may have spread it to Alcon.

His advice is full quarantine.  No relatives, hospital staff stay location. Burn all medical waste and bodies.  Recruit experienced ebola Docs from other areas.

In all honesty, the fact that those who had initial contact with Duncan ( first hosp visit, apartment, ambulance) still have not shown signs...yet.... Bodes very well for containment ( assuming someone seeks medical attention early stages). The bad part is obviously late stage is highly contageous. It sounds like staff was provided with either incorrect suits or ones that dont fit. Additionally, CIDRAPs recommendations should be considered for late stage handling of patients.
Ill fitting suits/and masks/ respirators can easily lead to infection in high contaminate areas. Ebola is handled ina BSL-4 lab for a reason. People wear fully encapsulated POSITIVE pressure suit....for some reason HCWs dont rate.

Providing incorrect equipment will result not only in HCWs becoming infected, but eventually result in HCWs refusing to work with such patients...this is where the big problem lies. If this happens, then ebola will get loose in the US beyond present area.

It appears that ebola continues to thrive and replicate even after the host victim is dead, to the extent that there are trillions of virus particles at that stage.

Rats know to flee a sinking ship, and fleas jump off of freshly killed animals. I wonder if somehow this virus knows it's time to leave its victim and find new digs. Certainly infectiousness is much greater near and after death.

Agree completely. The dead are a serious vector. I wouldnt be surprised if the poor saps assigned to remove the bodies are severely under equipped. Its the cleanup staff that typically are ill equipped and under trained. Support staff goes down and this will get ugly real fast. Ive seen numerous reports that many of the W.Af nations are merely telling infected to stay home and for families to treat in place.
As long as this stays 'contained' (relatively speaking) in the US we are ok, but if it pops up in 2 or 3 other cities I think systems will get over whelmed very quickly.

Dr. David Sanders of Perdue has been studying the Ebola virus since 2003. He stated very clearly on Fox News "The O'Reilly Factor" tonight (Tuesday, October 14th) that the Ebola virus CAN infect lung tissue.
