Nigeria Man Tested For Ebola In Hong Kong; Would Be First Case Of The Disease In Asia

With increasingly more cases - for now, largely isolated - of suspicious Ebola infections reported out of Africa and around the globe, it was only a matter of time before one of the world's most densely populated megapolises at 17,024 people per square mile, Hong Kong, raised the alarm as well. Which it did moments ago when Hong Kong's Centre for Health Protection said on Sunday that a man from Nigeria is being tested for the deadly Ebola virus at Princess Margaret Hospital. If confirmed, it will be the first case of the disease in Asia.

From Xinhua:

The 31-year-old who had been vomiting and suffering from diarrhea -- both symptoms of the disease -- arrived in Hong Kong on Thursday.

 

He was sent to the hospital from ChungKing Mansions where he had been staying at a guesthouse.

 

More than 900 people have died in the Ebola outbreak in West Africa. The World Health Organization has described the outbreak as a global emergency and the worst in four decades.

Hong Kong's RTHK adds, that the 31-year-old Nigerian man showed symptoms of vomiting and diarrhea after arriving in Hong Kong on Aug. 7. He was sent to Queen Elizabeth Hospital from ChungKing Mansions in Hong Kong’s Kowloon area, before being transferred to Princess Margaret Hospital.

The result of his blood test is expected in several hours, although one would wonder what is more important in this case: avoiding a wholesale panic (see Fukushima) or releasing the ever so slightly non-GAAP truth.

Meanwhile in the UK, border staff threaten to strike over risk of deadly Ebola entering the UK reports the Daily Star:

Border Force staff at ­London’s Heathrow and ­Gatwick airports are furious no measures have been put in place to preserve their health.

 

An insider told the Daily Star Sunday: “There is panic. It takes one person to come into this country with Ebola and all hell will break loose.

 

“Workers are being told to look for passengers who ­appear unwell and to raise the alarm immediately. But given how dangerous it seems to be, that doesn’t seem enough.

 

“People are scared and you can’t blame them. Many aren’t confident we are going to be kept safe and this has led to strike talk among some staff.”

Perhaps it is not too late to let TEPCO management deal with the Ebola epidemic considering their bang up job with Fukushima, or rather the cover-up effort, not anything else per se.

Nah, a real disaster would be something that is airborne, easily transmitted, is very deadly, and has a long incubation period so by the time anyone realizes there is a problem it is beyond an quarantine efforts. The WHO has been watching this from early. Letting it get out a bit with some news hype will be just enough to gather support for more surveillance and control of the world population.

"A local daily reported Tuesday that the nut Bitter Kola  had stopped the Ebola virus in test tubes in initial laboratory experiments, experts, including a Nigerian pharmacologist, revealed. 

"Meanwhile, Minister Nyenswah is calling on the public to report anyone or group of people administering vaccines supposedly for the prevention of Ebola or any form of drug or tablets, herbs or injection in the communities to the nearest police station"

"Assistant Health Minister for Preventive Services, Tolbert Nyenswah, has disputed [that Bitter Kola is a cure]"

So it looks like the Nigerian gov't knows who it is taking orders from. 

The people of Nigeria, too, are (rightly?) distrustful. "Ebola Virus Disease (EVD) was imported into Nigeria recently by an American-Liberian, Patrick Sawyer, [and since then] hand sanitizers, salt and bitter kola scarcity have hit the Federal Capital Territory."

SCMP--Chungking Mansions is a group of linked high-rises on Nathan Road in Tsim Sha Tsui. It is known for its South Asian restaurants, electronics shops and cheap guest houses that together have almost 2,000 rooms in total.

HONG KONG - A Nigerian man suspected of having contracted the Ebola virus has tested negative, said Hong Kong's Department of Health on Sunday night.

In a statement sent to the media, the department's Centre for Health Protection (CHP) said the 32-year-old man's "blood sample has been tested negative for Ebola virus upon preliminary laboratory testing".

"Since August 9, he has developed diarrhoea and vomiting but no fever. He attended the Accident and Emergency Department of Queen Elizabeth Hospital today and was transferred to Princess Margaret Hospital for treatment under isolation," added the statement.

"Initial investigations by the CHP revealed that the patient travelled from Lagos, Nigeria to Hong Kong on August 7 via transit in Dubai. In the past one month, he did not go to the other three affected countries (Guinea, Liberia and Sierra Leone). He had no contact history with sick persons or animals and did not visit health-care facilities. He is currently in stable condition."

I wonder how he traveled to HKG, as there are very few direct flights between Lagos and the Far East. Most probably on one of those Middle Eastern Gulf carriers like Emirates, Qatar or Etihad? Or perhaps via London with BA or Virgin?

Infection can also occur if broken skin or mucous membranes of a healthy person come into contact with environments that have become contaminated with an Ebola patient’s infectious fluids such as soiled clothing, bed linen, or used needles.

Anyone who has close contact with a person infected with the virus, or someone who handles samples from patients is at risk. This includes hospital staff, family members and laboratory workers, all of whom are at risk if they do not use proper barrier nursing techniques. These precautions include wearing protective gowns, gloves, and masks, in addition to wearing eye protection (eg eye glasses) or a face shield.

Infection generally does not occur through routine, social contact (such as shaking hands) with asymptomatic individuals. The likelihood of contracting any viral hemorrhagic fever (VHF), including Ebola, is considered very low unless there has been direct contact with the blood or body fluids (eg saliva, urine) of symptomatic infected persons or animals, or objects that have been contaminated with body fluids.

UR, the information above is on older variation of Ebola. That is the reasons the virus never turned into a pandemic due to the fact it was not very contagious. This version raging through Africa appears more contagious and that is why it is dangerous.

The West African strain of ebola has existed for at least 8 years, so it isn't new. However, something, yet to be disclosed, has changed about that strain in the past (now) 9 months since this outbreak began.

Those paragraphs are very clear if examined from a legal perspective (except as to the duration that virus can survive on an exposed surface), anything an infected person comes in contact with can carry the virus, and someone touching that surface and then touching their face (which is has number of mucous membranes and other points of entry) can infect themselves. The bullshit from the medical establishment about bodily fluids and difficulty of transmission is exactly that, bullshit, and counter-productive because it creates a false sense of security.

HONG KONG: A Hong Kong woman has tested negative for Ebola, a source at a hospital in the city said on Wednesday (July 30). The 39-year-old was suspected to have contracted the deadly virus as she fell ill after returning from a trip to Kenya.

 

here's hoping it's another false alarm.

although, if this new suspected EBV patient tests positive, then we more likely could expect more restrictions on flights going to and coming from a wider swath of african countries.

I have argued with medical professionals regarding quarantine. IMO, quarantine needs to be for 21 days or just close the borders to travel from the countries like the Saudi's have done.  But since our southern border is not secure, it is just a matter of time till there is a pandemic of some kind.  This chickie is not going to count on the govt or MSM to report actual news as I will go by my gut when I feel it is time to self quarantine.  As I live in a very rural area step one is in place.  Whether it be by weather,economic, or viral I try to ensure that I am self sufficient.

Viral hemorrhagic fevers (VHF) are acute infections with high case fatality rates, associated with the risk of nosocomial transmission (3). A rapid confirmation of the clinical diagnosis is therefore required by methods such as antigen capture enzyme immunoassay and serologic detection of immunoglobulin M. With the development of PCR technology, it has become possible to rapidly test for viruses that cause VHF (4510). We have recently confirmed a case of acute yellow fever with fulminating hepatic failure by reverse transcription-PCR (RT-PCR) (2). In spite of the very high viral RNA concentration in the plasma sample, it initially tested negative. A confirmation by PCR would have been missed if we had not tested a duplicate sample, inoculated with in vitro-transcribed yellow fever virus RNA, to detect substances that are inhibitory to RT-PCR (5). When this control reaction failed, we diluted the plasma in log10 intervals, reprepared RNA from the diluted material, and tested it by RT-PCR. The virus was clearly detectable in the patient's plasma diluted 1:100, 1:1,000, and 1:10,000 (Fig. ?(Fig.1).1). Again, no virus was detectable in the undiluted sample, and detection was also partially inhibited in the 1:10 dilution. Quantitative real-time RT-PCR with the diluted material yielded a projected concentration of >106 copies of viral RNA per ml of the undiluted plasma (Fig. ?(Fig.1).1). Interestingly, we have observed a similarly strong inhibition phenomenon with plasma from a moribund patient with acute Ebola hemorrhagic fever from Gulu, Uganda (Fig. ?(Fig.2).2). The viral RNA concentration in this case was 6.9 × 108 copies/ml. No hemolysis, which often causes inhibition of PCR (1), was observed in plasma from the two patients. In both cases, however, the patients were suffering from severe organ manifestation of their disease (aspartate aminotransferase in yellow fever sample, 15,000 IU; alanine aminotransferase, 6,000 IU).

I am suspicious this indicates the extremely high titer of Ebola is overwhelming the concentration of the primer. Primer and internal controls are competitive rxns (assuming they are using an internal control to detect inhibition and quantitation). There is existence of numerous sources of variation in RT-PCR including template concentration and amplification efficiency. Doing this for 15 years makes me appreciate the extreme sensitivity of this assay but the strong likelihood of false positives due to contamination.

Someone has to be quite courageous to work with wild type Ebola.

This would seem to indicate such. However, I wish they would define a " Low Risk Exposure". Are there documented cases of transmission in this manner? For Ebola, this indicates a new variant and would explain a lot of the increase of infectivity. Generally increases in infection rates are concurrent with a decrease in virulence which is what we are seeing as well. Not that I would take much heart in 60% vs 90% lethality.

So is this assuming droplet aerosols touching mucus membranes such as nose, mouth and eyes? This was not documented in previous outbreaks that I can find. How would this be determined without scientific documentation? This recommendation sounds like what would be for a standard airborne precaution as TB.

I am puzzled over the initial negative results. This virus is replicating logarithmically and PCR can detect just a few copies/ml. They must be using an Elisa methodology for screening which is far less sensitive.

The way to deal with this is to basically stop international travel for a period of time until no new cases are being confirmed. The problem with this approach is that it will set off the greatest domino effect in financial markets that we can imagine.

One way or the other we are screwed if this virus gains a foothold in the general urban populace which flies, drives and comes into contact with literally hundreds if not thousands of people a day.

I agree on the travel limitations.  To me, the biggest downside risk for economies/markets isn't limiting travel.  Its this thing getting a toehold in a developed country.  That is the tail risk here.  If airlines simply followed BA's lead and canceled flights to/from West Africa there would be little fallout in the markets.   In fact, the markets have been following this story nervously and there might actually be a relief rally of sorts.  Make no mistake - people are worried about this all around the globe.

If you are spewing bodily fluids because of Ebola infection, it is my understanding that you are further into the cycle than 8 days.  They are only testing people with symptoms at this point, so I am assuming those tested are fully loaded with the virus if thats what they got.
