This is how a spanish hospital is isolating an ebola-infected nurse

The spanish government has been dismantling public hospitals ever since they won the elections.

 

"It was reported at the outset that no protocols or realistic training to address this situation because they are procedures for which you must train: you can not teach a person to put on a suit that costs so much to put it in 20 minutes , "

This is the first case of ebola detected in Europe and the spanish government is to blame for it.

Major earthquake shakes southwest China, damage likely - LA Times

ficials in China have launched national disaster response teams after a strong earthquake hit the southwest of the country Tuesday night.

The earthquake, which was at least 6.0 in magnitude according to preliminary data, was centered about 11 miles west of Weiyuan, China, in Yunnan province.

The area borders Myanmar, Laos and northern Vietnam.

There was some conflict in the reported magnitudes of the quake; United States Geological Society data reported a 6.0 magnitude, while China’s Earthquake Administration said it was a 6.6 quake.

There was no immediate reports of fatalities or damage, but according to initial computer modeling from the USGS, significant damage is likely and the disaster could be “potentially widespread.”

According to USGS, people in the surrounding areas live “in structures that are highly vulnerable to earthquake shaking,” with the primary building type being unreinforced brick masonry and adobe block construction.

How did any cases appear in Europe in the first place ? Travel. Quarantine works. you just don't allow travel to and from any area with an un-treatable disease. You just don't do that. The Drager suits we used to use had full environmental control - via a feed and return umbilical. So, "shirtsleeve" environment all the year round, with individual flow temp / direction preferences set via a fully sealed sleeve-mounted membrane keypad. "StarTrek" technology, in the early 1980's.

No idea how much the MoD paid for these, but seeing as I'm still here after many exposures to Cat organisms (principally Marburg but also modified SV40), they obviously worked, and were more than comfortable anough to permit 4 hour or more shifts in the "Clean Rooms".

Parrotile, has there ever been an incident to your knowledge of a bio 4 agent infecting personnel using proper PPE in a properly outfitted lab? Our clinical labs do not have the capability to remotely handle this. Several coworkers have contracted Brucella using proper procedures. One colleague of mine died of meningococcemia and I handled the same isolate. Another top notch colleague in San Fran got Francisella tularensis and was very ill.

I cannot believe we are being so cavalier about this virus especially with no studies that definitively elucidate infectivity. I can look up the LD50 of many bacteria but who has done this type of research with Ebola?

I am finding it hard to find hard facts.The mechanisms of attachment and entry of the virus are not clear, but are assumed to be similar to those of other (-) strand RNA viruses, such as influenza virus. The replicative cycle of the virus is unknown, but likely involves a mechanism where the membrane proteins are produced at the ER, processed through typical cellular pathways, and sent to the membrane. The viral polymerase makes further mRNA transcripts and eventually begins replication of the virus, first making (+) RNA strands and then (-) RNA. Eventually, enough viral proteins and (-) RNA accumulate to allow viral assembly and then budding of the virus through the membrane. The mechanism of viral assembly and budding is yet to be elucidated. Every thing just seems so nebulous to me! And I'm supposed to handle this under a decertified laminar flow hood with a N95 mask?? REALLY. Am I nuts to be freaked out or has ZH turned me into a paranoidal conspiracy tin foil hat wearer? Help!

M'am - when working for the MOD we had a zero (0) lab. acquired infection rate. Say what you will about "Government" inflexible attitudes - their inflexibility where infection management was concerned, was a very significant safety factor.

The filoviruses are interesting. VERY ancient too (evidence of "paleo" filovirus sequences in many mammals - but may have evolved from some other common source). Seems they are particularly effective at hijacking cellular machinery, and we've no real idea as to the inoculum potential - quoting TCID50 lab values isn't much use in the field (though CIDRAP did excerpt a good article showing reliable (as in all-cases infection) of pigs using the Ebola Zaire variant, at 150 TCID50 which isn't that many virions), seeing as at least one mouse model suggests bloodstream circulating copies in the order of 100 billion per mL . . . http://www.ncbi.nlm.nih.gov/pmc/articles/PMC2657000/ Add in the ability of Ebola to block humoral signalling pathways and we would have a problem if this becomes more widespread - http://jvi.asm.org/content/73/4/3491.short

As mentioned before, here we handle "anything unusual" in isolators. Not glamorous ones, just ones that work, and so far we've not had any Clinicians or Pathologists contracting anything (even Noro - which spreads like wildfire at a Ward level - so much for effective use of PPE!). If you're wondering whether you can "afford" to treat Ebola patients - seems $1k per hour is the going rate - http://www.bloomberg.com/news/2014-10-07/bill-for-ebola-adds-up-as-care-costs-1-000-an-hour.html, and the cost of the "vaccine" is more like $100K per patient. Maybe you could use these figures to persuade your bean-counters to protect YOUR health, since the costs of "fixing it" outweigh even the most glamorous safety cabinets on the Market today! Hopefully you wear a FULL face shield along with the mask (aerosol splatter), and you work right at the very back of the cabinet!

Since management is going to be expensive. With the case count possibly (probably!) going to mount, it'll be interesting to see how the West copes once the "dedicated" facilities are occupied to our "usual" ED everyday levels of 105% occupancy rates. Tent hospitals the next step??

Thank you so much for this. The TCID50 is insanely low and 100 billion copies/ml? Dear God. I was trying to challenge our upper limit analytical range when we were doing manual HCV PCR and played with dilutions of a 100 million HCV standard. Yup, contaminated the lab with that little stunt ( all the days before UNG!) I'm sure there are still amplicons in the walls if I scraped them. Damn near died of bleach fumes for days. My record was an immunosuppressed patient with a viral load of 210 million copies/ ml HCV. HIV counts never are this high. Even if someone recovers, the organ damage must be extensive and permanent.

Thank you so so much for these informative articles and your perspective. I will add them to my collection. The mouse study was impressive but to make the assumption this would have a similar effect in humans? A bit of a stretch especially with such a small sample size. I can't see too many volunteers for such a study. This would have to take place as an experimental therapy at last resort. We shall all be guinea pigs.

The more I learn the more I fear this virus, if given the opportunity, will burn through us before an effective response can be made. I stand by my original assessment. Run.

I have realized that most of the country believes consesus is the same as science. The idea of years of hard work just to get a little useful data or applying the scientific method to expand knowledge simply isn't widely understood. I can relate. I struggled here for years to grasp the financial information being presented. Spent hours learning just basic concepts of economics. At some point I felt like flinging up my hands and begging someone to " tell me what to believe". It is just so complicated regular people just give up and hope someone can distill it down, simplify and summarize. It just isn't that simple unfortunately, too many variables are present and not enough data to make useful conclusions rather than vague conjectures.

Most here are wise enough not to believe the standard party line but the frightening part is no one really knows what this virus will do given the opportunity.

I have said since half of forever that the next crash will be caused by something almost nobody sees coming.  Something not on the radar, not visible in the worlds of finance.

More and more I believe that "something" is likely the shocking realization that much of the "hollowing out" of the west has already happened.  There really isn't as much resource in those long-trusted institutions as everyone thinks.  Our military, education system, medical system, etc.  ALL OF THEM ARE FAR MORE FRAGILE AND LESS FUNCTIONAL THAN THEY APPEAR.  All of them have become more or less government works programs and stocked with political cronies, ruining any effective value no matter how much money you throw at them.

Put these things under any unusual strain, coming from a non-traditional angle and they snap like a rotten stick.  Little strength, no flexibility.

Maybe Ebola is what snaps that stick.  Maybe not.  But the risk comes from believing your house is free of termites, when, in fact, it's infested with them.

For temporary suit repair (tears / cuts) Gaffer tape works fine, but the damaged suit must be removed from service after session completion and room decontamination, for permanent repair (permanent PVC patching).

If this is a "purpose built" containment / management area, both supply and return air terminals should be fitted with tandem HEPA (or preferably ULPA) filtration, with the means to isolate (and decontaminate in-situ) used filters.

Looking at the door design / absence of locking system / access control system, and absence of effective door seals, I rather suspect this is not what we're being told.

Had to take my wife to the hospital this morning for outpatient surgery. Everything went well. Alabama hospital folks were nice, polite and helpful - even at O dark 30. 

Sat in the waiting room and watched dozens of lard assed, morbidly obese, lumbering behemoths waller in and plop down for their "healthcare". 

The black female across from me was so fat that she couldn't tie her shoes. They had velcro straps. Sad thing is, she was so incredibly fat she couldn't even close the velcro straps  - her velcro strap shoes were unstrapped. 

And she wasn't the patient. She was driving the patient. 

Plenty of half crippled, 300 plus pound white trash as well. Had one fella straight out of deliverance try to talk to me. I grew up in Alabama and I couldn't figure out what the dumb shit was trying to say to me.

We were fucked long before Ebola ever showed up. 

We've got millions of people in this country who too fat to wipe their own ass, much less grow their own food or trap their own protein. 

It was disgusting and pitiful at the same time.
