Dallas Nurse Caught Ebola Because CDC Protocols Are Inadequate
Building Safer Protocols Isn’t Rocket Science … It’s Just Common Sense
A nurse in Dallas has caught Ebola even though she was wearing full protective gear.
The Centers for Disease Control says she must have broken protocol, or else she couldn’t have caught it. Maybe she did … or maybe CDC assumptions are overly-optimistic.
But the whole point of protocols for dealing with life-and-death situations is to have backup systems, redundancy and a margin of error in case something goes wrong.
In other words, if a mistake could be fatal, you don’t just hope that there’s no human error or natural accident. You build safety systems in so that – if something goes wrong – no one dies.
Safe Removal of Protective Suits
CDC head Frieden said today that removal of protective clothing is one of the easiest ways to get exposed to Ebola, if done incorrectly.
He also said that it is “not easy to do right.”
Yes …and the protocol should reflect those facts.
Specifically, the CDC protocol should require:
(1) Spraying of bleach or other disinfectant or uv light on the healhcare worker’s protective clothing before it is removed
(2) A buddy system, where an infectious disease specialist helps the healthcare worker take off their protective clothing without exposing themselves in the process
Respirators
Even the CDC now admits that Ebola can be spread if a carrier coughs or sneezes into the face of a healthcare worker.
And numerous scientists say that Ebola can be spread via aerosols created by vomit or the flushing of a toilet.
As such, CDC protocols must require frontline healthcare workers treating Ebola patients to wear respirators.
Phone Screening
Doctors should not have to guess whether patients have just come from Ebola hotzone countries like Liberia, Sierra Leone or Guinea.
Receptionists at doctor’s offices and hospitals around the country must ask the patient on the phone – before they come in – whether they’ve recently traveled there.
If the answer is yes, extra caution should be used to examine the patient … or they should be sent to specialist facilities which know how to spot and handle potential Ebola patients.
Postscript: We think a travel ban from hotzone countries should be enacted. But if we're not going to do that, let's at least have a real screening test ...
30-Minute Test
A majority of Americans support banning all flights to the United States from countries experiencing an Ebola outbreak.
Screening people at West African airports with a thermometer can’t work, and is just for show.
But Japanese scientists have developed a test which can determine if someone has Ebola within 30 minutes. And the test is cheaper than the one currently being used in West Africa.
So – if we’re going to continue to allow folks from West Africa to fly into our country – why don’t we demand that they get tested for real?
A health agency like the World Health Organization, UN or Centers for Disease Control should buy the international airports in the hotzone countries one of the Japanese testing kits.
Then all travelers should be tested while they’re waiting for their flights.

Do we know if Duncan's family has symptoms? Has anyone heard any reporting on the family or phone interviews with them, or anything? If that info is out there, I've missed it.

Her father came down with it, and was last photographed dying on the front porch. Duncan rented a room there. As of Oct. 2, regarding those who were in contact with the woman, "Already nine others are dead or dying. All the neighbors have been put quarantined."

Also: The virus makes a copy of itself once it's in a human. It does this via RNA, which is notoriously sloppy. So when it replicates itself, it does so with errors. I'm not a health care professional, but I think this means there are lots of different variants of it, some less lethal, some more.

From what Frieden said today, it sounds as if they are respecting medical privacy. If the patient and doctor don't want information to be released, it won't be. I believe what he said was "it's up to the patient and doctor."
The observation of privacy for the patient could have something to do with it. However, that sure does not seem to keep reports of "another possibly infected person" from getting in the news. The privacy concerns seem to be carried out by not naming the person, not by failing to report the incident of symptoms.

I'm following this locally. I've read that the CDC had interviewed the nurse and concluded that there some inconsistancies with regards to what she was using for her PPE and how it was used - still focusing on the nurse being responsibe for infecting herself. More recently the CDC is walking back from that position. They are now worried that the protocol was inadequate and is going to be changed. They are sending people from CDC to train the staff at Presbyterian. As a result of the inadequate protocol they expect more staff to test positve for the disease.
This begs the question. Was the CDC on site at Presby throghout Mr. Duncan's stay? If so, why would there have ever been a breach in protocol that need to be investigated? If not, WHY THE FUCK NOT. Was there something more pressing than the first case of Ebola in the US being initally mishandled at a community hospital? Why was there not a CDC person there 24/7 watching everyone dressing and undress when entering and leaving the room?

I think Friedan's comment that there could well be more infections among the Texas staff was a dead giveaway that he thinks the protocols are insufficient. (That's the only reason to make such a confident assertion.)

Why no pictures of bleeding ebola patients ANYWHERE? This is a horrible disease which late in stages causes bloody vomit, diarrhea and profuse bleeding from the eyes and ears. But not a single pic of it anywhere? Especially on the "news" channels showing "late stage" patients walking around appearing quite healthy. Not even sweating? We have no problem showing kids getting their head blown off in Gaza, but Ebola, ISIL beheadings and Sandy Hook 911 calls are too gruesome. Think about that for a moment.

Look, bleeding is end-stage and only in about 50% of the patients. It is only extreme in an even smaller percentage. The virus itself attacks the immune system, in some ways similar to AIDS. It attacks the cells that identify pathogens, removing the body´s ability to target it. The result will be eventually random unspecified hemmorhages, but more often, complete system shock occurs first, causing organ damage and killing the victim. Not everything is a conspiracy. Ebola is the real deal. The only conspiracy here has been the effort to avoid panic by insisting that everything is ok and this is not problem. Why? Because the economic consequences of panic and people sheltering in place and hospital workers and first-responders walking off the job are not pretty to contemplate. So instead of leading, they lie. Only about 1 in 5 Ebola patients has the bleeding. I presume the others die first of organ failure. And if you are late stage enough to be bleeding (it is a late stage thing) you won't be "walking around appearing quite healthy."

Americans are about to hit a panic button. Then all civilities and Home Land Security Agency will be totally irrelevant.
For a local panic button, pretty much. The hospital is basically empty of normal patients that would go into the same building that Mr. Duncan resided in, and the hospital closed its emergency room for lack of patients. Business-wise, they're in deep trouble since the ER is the core "business."
In a widespread sense, I'm not convinced that enough people are beyond the "complaint" stage yet. My money is on it happening when people can't afford to watch Monday Night Football anymore.
