Most US Hospitals Cannot Safely Handle Ebola Patients

This Ebola outbreak is being called the “most severe, acute health emergency seen in modern times“, and the U.S. health care system is completely and totally unprepared for it.  The truth is that most U.S. hospitals are simply not equipped to safely handle Ebola patients, and most hospital staff members have received little or no training on Ebola.  And the fact that Barack Obama and our top public health officials are running around proclaiming that Ebola is “difficult to catch” is giving doctors and nurses a false sense of security.  There is a reason why Ebola has been classified as a biosafety-level 4 (BSL-4) pathogen.  It is an extraordinarily dangerous virus, and there are only a few facilities in the entire country that are set up to safely handle such a disease.

The Ebola patient that recently died in Dallas was the first to be cared for in a facility that did not follow biosafety-level 4 protocols.  And so it should not be a surprise that this is the facility where transmission happened

Of the six Ebola patients treated in the U.S. before the health worker’s case, Duncan was the only one not treated at one of the specialized units in several hospitals around the country set up to deal with high-risk germs.

 

The CDC’s director, Dr. Thomas Frieden, has said that any U.S. hospital with isolation capabilities can care for an Ebola patient. But his stance seemed to soften on Sunday, when asked at a news conference whether officials now would consider moving Ebola patients to specialized units.

 

“We’re going to look at all opportunities to improve the level of safety and to minimize risk, but we can’t let any hospital let its guard down,” because Ebola patients could turn up anywhere, and every hospital must be able to quickly isolate and diagnose such cases, he said.

The head of the CDC continues to underestimate the seriousness of this disease.  His opinion that just about any U.S. hospital can safely handle Ebola patients is being contradicted by a whole host of medical experts, including ABC News chief health and medical editor Dr. Richard Besser

Besser said he does not agree with the Centers for Disease Control, which says any U.S. hospital can safely care for an Ebola patient.

 

To do it safely, health care workers need to train and practice using protective equipment like they have been doing at the Emory and Nebraska facilities,” he said, referring to special biocontainment units at Emory University Hospital in Atlanta — where Fort Worth physician Kent Brantly was treated for Ebola exposure; and the Nebraska Medical Center in Omaha, where an NBC photojournalist is currently being cared for. “I would never have gone into an Ebola ward in Africa without being dressed and decontaminated by experts — health care workers here should expect no less.”

And even if our hospitals had the proper equipment and hospital staff were being given proper BSL-4 protective clothing, the reality of the matter is that most of them have not received adequate training.  Just check out the following excerpt from an NBC News article that was posted this week…

Three out of four nurses say their hospital hasn’t provided sufficient education for them on Ebola, according to a survey by the largest professional association of registered nurses in the United States.

 

National Nurses United has been conducting an online survey of health care workers across the U.S. as the Ebola outbreak has widened globally. After a Texas nurse who cared for the first patient diagnosed with the Ebola in the U.S. tested positive for the virus Sunday, the group released its latest survey findings.

 

Out of more than 1,900 nurses in 46 states and Washington D.C. who responded, 76 percent said their hospital still hadn’t communicated to them an official policy on admitting potential patients with Ebola. And a whopping 85 percent said their hospital hadn’t provided educational training sessions on Ebola in which nurses could interact and ask questions.

If this is indeed the most serious health emergency in modern times like the WHO is saying, then we need to get our health care personnel trained to face it immediately.

Sadly, if a major Ebola pandemic does break out in this country, there is no way that we are going to have the resources to be able to deal with it.

As I discussed yesterdayWND is reporting that there is only one BSL-4 care facility in the entire nation that is available to treat the general public…

Have you wondered why Ebola patients are being sent to Omaha, Nebraska?

 

It’s because one physician, Dr. Philip Smith, had the foresight to set up the Nebraska Biocontainment Patient Care Unit after the Sept. 11 attacks as a bulwark against bioterrorism. Empty for more than a decade, used only for drills, it was called “Maurer’s Folly,” for Harold Maurer, former chancellor of the University of Nebraska Medical Center.

 

The unit has a special air handling system to keep germs from escaping from patient rooms, and a steam sterilizer for scrubs and equipment.

 

It could handle at most 10 patients at a time, but one or two would be more comfortable, owing to the large volume of infectious waste.

 

It is the largest of only four such units in the U.S., and the only one designated for the general public.

If the outbreak in the United States is limited to just a few patients we will probably be fine.

But what if it isn’t?

Meanwhile, the Obama administration continues to do next to nothing to prevent more people infected with Ebola from traveling into this country.

Obama says that there is “extensive screening” at our airports, but that simply is not true.

The following is one example of the “extensive screening” that is taking place…

The World Health Organization is sending doctors to countries where the virus is most prevalent — Liberia, Guinea, Sierra Leone and Nigeria. Fusion’s Jorge Ramos spoke to one of the doctors, Dr. Aileen Marty, who recently returned home to Miami after spending 31 days in Nigeria. She says she was surprised what happened when she arrived at Miami International Airport.

 

“I get to the kiosk…mark the fact that I’ve been in Nigeria and nobody cares, nobody stopped me,” Marty said.

 

“Not a single test?” Ramos asked her, surprised.

 

“Nothing,” Marty answered.

And the head of the CDC continues to rule out a ban on air travel for non-essential personnel to and from the countries where Ebola is raging…

Dr. Frieden strongly argued against curtailing travel to and from West Africa, in part because that could make it harder to get supplies to those countries. “That will make it harder to stop the disease,” he said. “Whatever we do, we won’t stop travel to and from these countries.

It is hard to put into words how foolish this is.

If this virus gets loose inside the United States it could easily become the worst health crisis our nation has ever seen.

The key is to keep the virus from getting into our country in the first place.

Banning air travel for non-essential personnel to and from Sierra Leone, Guinea and Liberia would not be that big of a deal.  Many other countries have already done it.

But the CDC and the Obama administration are not even considering it.

If they have made the wrong call on this, it could end up costing large numbers of Americans their lives.

I wonder where the other three are and how much security has increased for these locations.  They should make public a list of people who are pre-approved for treatment.  You know, just in case you aren't sure if you are on the list.

This argument is getting old.  The Spanish Flu had one of the highest mortality rates ever seen for influenza - at 2.5%.  It killed between 20 and 40 million people in 1918 - nearly a million in US after returning WWI soldiers brought it back with them from Europe. 

Do you really want to equate influenza with Ebola (>50% mortality)?  Are you really that dense?

  The hospital I work for recently unveiled it's strategy regarding Ebola.  Ask "have you or someone you know been to West Africa recently?" for admission screening, and monitor for temps >101.5.  Those suspected of infection are placed in isolation.  Not level 4 biohazard iso, standard iso. Calling this inadequate, is a serious understatement.  The facility intends to follow CDC recommendations. 

  Many of the direct care providers I talk to (those who have done their homework),  understand how serious this is.  Coming into contact with infected people without adequate precautions is insane.  We would have to be idiots to watch the level of protection used in Africa, and believe we need less for some reason.  Strict decontamination protocols are used to minimize risk, along with appropriate PPE, yet many nurses and doctors have contracted the disease and died in Liberia, and elswhere. 

  The CDC is derelict in it's duty to the public.  They can't possibly believe what they are telling us.  As for their claim that shutting down non-essential flights to and from the trouble spots will make the problem worse, bullshit.  Someone's need to travel to see family, etc., doesn't supercede the need to limit the spread of Ebola.  As the author points out, other countries have already done it. 

  In the end, it may be the southern border that proves to be our undoing.  No screening process there.  Traveling from/through Mexico with Ebola in cramped quarters, with enough incubation time to make it to any number of states, infecting who knows how many in the process.

Not only tried..but sued...just think of the liability for a nurse following a hospitals protocols....and catches it and dies or even lives...I think life afterwards is going to be very tough....it destroys your organs.....anyway..the actual costs for a case are over $500,000.....and liability is much more than that....and soon the airlines will be hit with those suits too.....but yes...just a couple more first responders catching it ...and there will be no more first responders.....I would not show up....not my job man....to die for some illegal in my country....

my wife is telling me the nurses are going to raise hell if the hospital insists upon treating ebola without the proper protocol in place. that is the technicality in whether ebola is aerosol or not. if it is not then most hospitals have the protocol and facilities in place. if ebola is aerosol then the number of hospitals with the proper facilities and equipment is dramatically reduced. that poor girl in texas was sickened by political wankering.

Most nurses are abused by administration & are used to working in conditions that threaten the safety of themselves & their patients on a daily basis. Working without the proper equipment or 'protocol' is nothing new to them. When you have these issues of inadequate supplies & improper staffing at the most basic level, the reasons why being a discussion for another time, expecting a positive pressure suit to provide patient care under strict isolation measures is unfortunately quite unrealistic .

It is true that its not just about having a job & it is about survival.
But healthcare workers tend to have a bit of a sacrificial ideology about them. Meaning even without the proper equipment they will put themselves at risk & place the patients needs above their own, as they've been doing throughout their careers.

I'm guessing we know now for whom those 160k suits were for .

nurses sacrifice more of themselves in a week than most people sacrifice in a lifetime. the sociopath in me thinks they are nutz but the reality is their devotion to their patients, even the nurse ratchets, is an incredible testament to the goodness of man(i don't think it is a mistake that most of them are women)......and yes, admin constantly tries to take advantage of their inherent goodwill to make more money.

Since we have less than 10 people, in America, that have Ebola, we have time to bring our people and facilities up to task. The biggest problem will be WHO, CDC, NIH have never been able to pull their heads out of their asses, even for something like lyme disease. I have little faith they'll pull their heads out of their corrupt, lying, stuck-in-yesterday, asses for Ebola.

Actually, you don't want to "Handle" them at all, dumb-ass MFers!

You keep them at pole's length and "treat" them from a distance:  1. Kinetic injections of Pb help contain the spread, and 2. Jets of organic compounds at Fahrenheit > 451 will cure and eliminate it.

Proper treatment requires dialysis, blood transfusions, intubation/respirator, and all done in intense ICU isolation settings.  Dialysis for one patient can take hours. How many extra dialysis machines do you think are available?  How many extra dialysis machines are available in the conditions above?  It's not going to take very many Ebola patients to overwhelm the system.

Use dialysis on an Ebola patient & you can (& should) say goodbye to that dialysis machine.

Who in their right mind would be willing to use that same machine on another patient after running an EBV invected blood through it ? & could it even be properly sterilized ?

Sorry to say but I think by the time the EBV pt needs dialysis it is probably too late already ...

I run these machines. The blood does not go through them. In simple terms, the machine just moves fluids. It circulates blood and dialysis fluids through a disposable closed circuit that the machine moves through the tubing by a series of compressive wheels. As to properly cleaning it, I do not know. You can wipe the external surface but not the fan cooled interior parts.

Any potential for contamination of interior parts that can be cause for concern for later patients? What is the standard for BBP/HIV/HEP+ ?

If the interior were not the issue, would you yourself feel safe using & touching this machine after an EBV+ patient? Knowing it's virulence & PPE requirements

Since the people close to Duncan have not contracted the disease, it seems that the big risk is to medical personnel that have repeated, close contact to patients.

The major thrust of the program to combat the disease should be detailed training on protective coverings-including removal, handling of items, disinfection of all protective gear and access  to the best protective gear for all medical workers dealing with infected patients. 

The US medical establishments should protect their labor forces with no expenses spared.

Nice thought but a big issue is the physical layout of nursing units. Most are not designed to safely handle such dangerous medical waste, let alone decontamination of workers. My facility is less than 10 years old and we have to carry medical waste through clean areas to get to the waste storage area for the unit. Then, when the cans are full, they are dumped into a bulk cart and wheeled through our clean area and then the rest of the building to the dumpster area. We are currently not at all up to the task.

Okay, here is the truth I take away from this.

They are not telling the truth about it being airborne because they don't know how to pay for proper protection for everyone. 

Remeber Fight Club, where the guy is an agent who goes around seeing if cars need to be recalled or not?

They will accept a few deaths because they can afford the insurance payout.

If it will be too expensive, they do the recall.

I suggest that is what is going on here. They can afford level 3 protection and a few deaths. They can't afford level 4. 

I admit this is a guess, but this is how it all adds up to me. They are deciding they can do this with a few deaths, still keep it under control and SAVE MONEY.

That is your damn conspiracy.

Hospitals are not equipped to deal with an illness that is spread through coughing, puking, and bleeding? Now I'm scared. What happens if someone is admitted with something more easily spread, like the flu? Does everyone in the building get infected? That's a death sentence for anyone with an impaired immune system, such as someone with cancer or AIDS. I think surgery messes with your immune system too. You really don't want to get the flu after you've had heart surgery.

I guess the lesson here is to stay the hell away from hospitals. They can't even deal with ebola, which should be relatively easy to contain compared to other illnesses.

neither.

I could have given you my expert opinion on the likelyhood of US hosptals

containing anything,except dying people, a lot cheaper than the CDC.

description of what a bsl-4 virus is and the handling precautions for it.

Note the main points.
- Ebola is a bsl-4 virus
- A bsl-4 virus has a high individual risk of aerosol transmitted laboratory infections
- PPE required to handle it is fully encapsulated gas tight suits with their own independent air supply to the wearer.
- Full array of different isolations and decontamination procedures to ensure 100% safety to people operating in that environment.

Something is very wrong with how the authorities are handling this, and they know it!

First rule is isolate the risk of the contagion coming into your country.

So what do they do. Fly it straight in!

Biosafety level 4

The Galveston National Laboratory BSL-4 (P4) lab on the Campus of the University of Texas Medical Branch
This level is required for work with dangerous and exotic agents that pose a high individual risk of aerosol-transmitted laboratory infections, agents which cause severe to fatal disease in humans for which vaccines or other treatments are not available, such as Bolivian and Argentine hemorrhagic fevers, Marburg virus, Ebola virus, Lassa virus, Crimean-Congo hemorrhagic fever, and various other hemorrhagic diseases. This level is also used for work with agents such as smallpox that are considered dangerous enough to require the additional safety measures, regardless of vaccination availability. When dealing with biological hazards at this level the use of a positive pressure personnel suit, with a segregated air supply is mandatory. The entrance and exit of a level four biolab will contain multiple showers, a vacuum room, an ultraviolet light room, and other safety precautions designed to destroy all traces of the biohazard. Multiple airlocks are employed and are electronically secured to prevent both doors from opening at the same time. All air and water service going to and coming from a biosafety level 4 (or P4) lab will undergo similar decontamination procedures to eliminate the possibility of an accidental release.

Agents with a close or identical antigenic relationship to biosafety level 4 agents are handled at this level until sufficient data are obtained either to confirm continued work at this level, or to work with them at a lower level.

Members of the laboratory staff have specific and thorough training in handling extremely hazardous infectious agents and they understand the primary and secondary containment functions of the standard and special practices, the containment equipment, and the laboratory design characteristics. They are supervised by qualified scientists who are trained and experienced in working with these agents. Access to the laboratory is strictly controlled by the laboratory director.

The facility is either in a separate building or in a controlled area within a building, which is completely isolated from all other areas of the building. A specific facility operations manual is prepared or adopted. Building protocols for preventing contamination often use negatively pressurized facilities, which, even if compromised, would severely inhibit an outbreak of aerosol pathogens.
